New Student Forms

As you prepare for your transition to Geneva College, there are a few steps and several forms that you will need to complete and submit to Geneva College to ensure your transition is a smooth one. All the required forms are listed below and must be submitted by May 31, 2012 for the fall semester or by December 15, 2012 for the spring semester.

Be sure to note specific instructions regarding the health forms. In order for them to be considered valid, they must be completed correctly and by the appropriate physician.

Math Proficiency Challenge Exam

Math proficiency is determined by SAT or ACT score. Proficiency is demonstrated by a math SAT score of 470 or above, or a math ACT score of 19 or above. Students entering Geneva College in the fall without math proficiency may take the math proficiency challenge exam in the Science and Engineering Building, Room 341, at 3:30 p.m. on Friday, June 15, or Tuesday, June 19; or at 11a.m. on Wednesday, August 22. The fee for the math proficiency challenge exam is $25 and is to be paid at the door on the day of the exam.

Reading Assessment

Students admitted to Geneva College on probationary status are required to take the Reading Improvement I course to strengthen reading and study skills. Students entering in the fall may, however, test out of this course by successfully passing the Nelson-Deny Reading Test offered in Northwood Hall at 8 a.m. on Friday, June 15, or Tuesday, June 19; or at 2 p.m. on Wednesday, August 22.

Spanish Assessment

Students interested in taking the second level of Spanish (SPA 102) or higher should take the Spanish Assessment offered in Northwood Hall at 8 a.m. on Friday, June 15, or Tuesday, June 19; or at 4:15 p.m. on Thursday, August 23. More information

Health Forms

Completion of pages 1, 2, 3, 4 and 7 by the student is a mandatory requirement for matriculation to Geneva College. The examining Physician is to complete and sign pages 5 and 6 (Physician's Health Evaluation and Immunization Record), review pages 2, 3 and 4 and sign page 4 (Medical History Report). Attention Student Athletes: All student athletes need to complete this additional medical history report.

Health Insurance Information & Waiver

Health insurance is required of all students. Should you decided not to accept coverage from Geneva College, it will be necessary for you to provide a Health Insurance Waiver proof of coverage. Due to the healthcare law changes, the finalization of our student healthcare plan has been delayed.  As soon as our health insurance provider makes their waiver available we will add the link here.  Although the New Student Forms are due May 31, the Health Insurance Information & Waiver will not be due until June 30.

FERPA Disclosure

The Family Educational Rights and Privacy Act (FERPA) Disclosure Form is a form that grants permission for Geneva College to share upon request or upon the discretion of Geneva College, educational record information with the parents with whom students make their permanent residence. Please review form for additional information.

Housing & Roommate Preference 

This form is designed to give insight in roommate placement. Room assignments cannot be made or finalized until the Candidate Reply Form is completed and received with your deposit.

Meal Plan Selection - freshmen

Resident Freshmen will be required to choose one meal plan option. Three options with different levels of flexibility are available for resident freshmen. If a resident student does not designate a choice of meal plan in writing, a 21 meal plan option will be automatically assigned. This form is for New Students Only! Current students must make all meal plan changes directly through the Residence Life office.

PISGAH Program

New students will engage in low challenge course activities that focus on teamwork, communication and bonding In order to participate, the insurance provider of Geneva College requires that the following informed consent release be signed. Vehicle Registration - freshman Freshman Students are discouraged from having a car on campus during their first year. However, if you believe that it is in your best interest to bring a car to campus, and you wish to purchase a parking permit, you will need to bring your completed Vehicle Registration form and Vehicle Registration Card to Check In. At that time you will also pay the freshman parking fee to the student accounts office. Freshman Vehicle Registration forms mailed to the Admissions Office will not be accepted. Download form
Vehicle Registration - upperclassmen (more than 23 credits) or commuters

If you wish to purchase a parking permit you will need to bring your completed Vehicle Registration form and Vehicle Registration Card to Check In. At that time you will also pay the parking fee to the student accounts office. Vehicle Registration forms mailed to the Admissions Office will not be accepted.
