Sports Medicine

The Geneva College Sports Medicine staff is responsible for your injury prevention, care, and safety while participating in athletics. In order to provide the appropriate care upon your arrival and during your stay on campus we request your cooperation with the following items. Should you have any questions regarding the submission of paperwork, please contact the athletic training staff directly.


Returning Student Athletes:

  • Yearly physical exam: Returning student athletes will have a yearly screening at Geneva prior to the start of their season. The cost of the exam is $20. Cash or check will be accepted. (Checks made payable to: ASP Orthopedics) Student athletes must be cleared by our team physician in order to participate in athletics at Geneva College. No outside physical exams will be accepted. If a student athlete is unable to attend the scheduled physical time, he/she will need to make an appointment with our team physician via the athletic training staff prior to being cleared for participation.
  • SportsWare Online: Each year student athletes must ensure their personal, health history, and insurance information is up to date on the SportsWare online program. Returning student athletes should log onto SportsWare using the username and password they created and click through all tabs to check their information. Please make any necessary changes to ensure your information is correct (phone number, address, insurance information, etc.) Please contact the athletic training staff if you have trouble logging into the program for assistance.
    • Please make sure you have a current copy of your primary insurance card (both front and back) uploaded into SportsWare online via the attachments tab.
  • First Agency Form must be filled out entirely and turned in at the time of physical exam.

New Student Athletes (incoming freshmen/transfers):

  • Yearly physical exam: Each incoming and transfer student athlete will be given an in-depth physical exam by our team physician and his colleagues once they arrive on campus. The cost of the exam is $20. Cash or check will be accepted. (Checks made payable to: ASP Orthopedics) Student athletes must be cleared by our team physician in order to participate in athletics at Geneva College. No outside physical exams will be accepted. If a student athlete is unable to attend the scheduled physical time, he/she will need to make an appointment with our team physician via the athletic training staff prior to being cleared for participation.
  • SportsWare Online: Each year student athletes must ensure their personal, health history, and insurance information is up to date on the SportsWare online program. New student athletes, please read the initial log on instructions.
    • Please make sure you have a current copy of your primary insurance card (both front and back) uploaded into SportsWare online via the attachments tab.
  • First Agency Form must be filled out entirely and turned in at the time of physical exam.
  • Program Acknowledgement form must be signed by the student athlete and his/her parent/guardian and uploaded into the attachments section of SportsWare online.
  • Assumption of Risk, Consent to Treat, HIPPA form must be signed by student athlete and uploaded into SportsWare online via the attachments tab.
  • Sickle Cell Trait Testing: New legislation from the NCAA requires all student-athletes to provide confirmation of sickle cell trait status. Under the proposal, schools can confirm the sickle cell trait status of student-athletes before participation in intercollegiate athletics in one of the following manners.
    • Documentation – The student-athlete may provide documentation results of a sickle cell solubility test administered before participation.
    • Pending Documentation – A student-athlete who has taken a sickle cell test solubility test but whose results are not yet confirmed may participate provided the student-athlete engages in mandatory education, signs a waiver and receives appropriate precautions as set forth by the institution
    • Opt-out – The student-athlete may sign a Sickle Cell Trait Waiver Form declining confirmation of sickle cell trait status. Before signing the waiver, the student-athlete shall be provided education regarding the implications of exercising the waiver option.
    New student athletes must review the Sickle Cell Information sheet and provide documentation via SportsWare Online.


    NCAA Drug Testing:

    Any student-athlete that is currently taking prescription medications for Attention Deficit Disorder, Anxiety, or Depression must have a letter from the prescribing physician ON-FILE in the athletic training room by completing the ADHD Examination Form. The medications normally prescribed for the above conditions can result in a positive NCAA drug test. The information is required by the NCAA to be On-File prior to any drug testing administration as athletic eligibility can be affected by a positive drug test. Please follow the instructions to complete these additional requirements if you meet the above description.

    All student-athletes are eligible for random drug testing by the NCAA. In order to protect yourself and remain eligible to participate in collegiate athletics, please read the list of NCAA Banned Substances.

The cardiovascular science program is accredited by the Commission on Accreditation of Allied Health Education Programs (CAAHEP).
