ALL Student-Athletes:

In order to be cleared for participation in athletics at Geneva College, all student-athletes must complete all forms and online information prior to the start of the fall semester OR prior to the first day of pre-season practice, whichever comes first.   All student-athletes must receive a physical from their primary care physician using the Geneva College Pre-Participation Physical Examination Form. No other forms will be accepted. The pre-participation physical exam should be completed in June or July and is only valid until May 31st of any year.

All medical forms and information must be completed and submitted using the SportsWareOnline program and will not be accepted in paper form or via email or fax (except for the ADHD Examination Form). It is important that student-athletes write down their log in username and password in a secure location because each year they participate in athletics at Geneva College, they will be required to log on to the system and update their information. Athletic Medical Insurance and Important Forms

ALL student-athletes must complete the following forms annually AND submit a photocopy of their PRIMARY INSURANCE CARD (both front & back):

NCAA Drug Testing:

Any student-athlete that is currently taking prescription medications for Attention Deficit Disorder, Anxiety, or Depression must have a letter from the prescribing physician ON-FILE in the athletic training room by completing the ADHD Examination Form. The medications normally prescribed for the above conditions can result in a positive NCAA drug test. The information is required by the NCAA to be On-File prior to any drug testing administration as athletic eligibility can be affected by a positive drug test. Please follow the directions to complete these additional requirements if you meet the above description.

All student-athletes are eligible for random drug testing by the NCAA. In order to protect yourself and remain eligible to participate in collegiate athletics, please read the list of NCAA Banned Substances.

Sickle Cell Trait Testing:

The NCAA has required that all student-athletes complete a Sickle Cell Trait Solubility Test. All student-athletes must review the Sickle Cell Information sheet and provide documentation of test results via SportsWareOnline.
