Application for Chapel Exemption

All of the following information must be filled in for this form to be considered.

The chapel program is designed as an integral part of the College's educational mission and will serve as a weekly devotional time for the entire College community. Chapel will take place each Wednesday from 10:15-11:00 a.m. in the field house and is designed to provide opportunity for the campus to gather together to praise God, hear His word, and seek His favor and direction on a regular basis. All full-time students are required to attend Chapel. However, if you need an excuse from Chapel for any of the reasons listed below, please check the appropriate line, fill in any details and submit this form to the Campus Ministries Office.

Schedules are pulled 2 weeks after classes begin, so Chapel will still show up on your schedule. You will be contacted if there is a problem with your Chapel exemption.
