New Student Health Information


All incoming resident and commuter undergraduate and graduate students must complete the Geneva College Health History Form and Immunization Form prior to enrollment. These can be found by logging into MyGeneva and choosing Student Information > New Student Forms from the menu.

Please note: No physical examination is needed prior to enrollment except for student athletes.

Required Immunizations

The following immunizations are required for all incoming resident and commuter, undergraduate and graduate students (part-time, full-time, auditing classes, or transferring from another college or university who will be physically present on Geneva College’s campus. 

  • MMR (Measles, Mumps, Rubella) – 2 Doses 
    • 1st dose – administered at 12-15 months  
    • 2nd dose – administered at 4-6 years of age 
    • OR may submit laboratory evidence of immunity (titer) 
  • Meningitis (Meningococcal Quadrivalent – ACWY) – 2 Doses 
    • Required if living on campus or must have a signed waiver on file declining the vaccination  
    • 1st dose – administered at 11-12 years of age 
    • 2nd dose – administered at 16 years of age 
    • If initial dose was given after age 16, no booster dose is required 
  • Tdap – one dose in the past 10 years 
  • Varicella – 2 Doses 
    • 1st dose – administered at 12 months or older 
    • 2nd dose – administered at least 3 months after the first dose up to 13 years of age. 
    • OR may provide documentation of disease or laboratory evidence of immunity (titer) 

 
Guidelines for Tuberculosis Screening

  • ALL international students must receive tuberculosis screening from their health care provider to confirm that they do not have active TB BEFORE arriving in the United States (within 6 months of arrival). 
  • An interferon gamma release assay test is preferred over a tuberculin skin test for non-US born persons 2 years of age or older.  
  • All other students must complete the online Tuberculosis Risk Assessment Form.  The form asks 3 questions: 
    • Were you born in, lived in, or traveled to a country with an elevated TB rate for at least 1 month? (Includes any country other than the US, Canada, Australia, New Zealand, or a country in western or northern Europe.)
    • Do you have any current or planned immunosuppression such as HIV infection, organ transplant recipient or treatment with a TNF-alpha antagonist, steroid use (equivalent to prednisone dose of 15mg/day for one month or longer), or other immunosuppressive medication? 
    • Have you been in close contact with someone with infectious TB disease during your lifetime? 
  • Students who answer “Yes” to any of the 3 questions will require a tuberculin skin test and/or an Interferon Gamma Release Assay blood test (QuantiFERON Gold) before the start of the semester.
